De novo arteriovenous malformations following implantation of the HeartMate II left ventricular assist device.

چکیده
The HeartMate II (HMII) is the most popular continuous-flow left ventricular assist device (CF-LVAD) employed worldwide. Implantation of CF-LVADs has been associated with high rates of gastrointestinal bleeding, particularly from arteriovenous malformations (AVMs) [1,2]. Part of the increased bleeding risk relates to a reduction in circulating von Willebrand factor multimers [3]. However, it has also long been hypothesized that loss of the physiologic pulse, as occurs with CF-LVADplacement, may lead to hemodynamic changes in the gastrointestinal mucosa that are ultimately “angiodysplasia-genic” [4]. A 65-year-old man with no previous history of AVMs or gastrointestinal hemorrhage underwent uncomplicated implantation of a HMII device. He presented to the hospital 5 weeks postimplantation with frank hematochezia. After stabilization, he underwent both upper and lower gastrointestinal endoscopic evaluations, which revealed several large cecal AVMs with stigmata of recent bleeding (●" Fig.1), as well as numerous gastric antral AVMs without signs of bleeding. These AVMs were treated successfully with argon plasma coagulation (APC) therapy. He also underwent video capsule endoscopy, which revealed a single 4-mm, nonbleeding AVM in the jejunum, which was not treated. Once he had been stabilized, the patient was discharged home. He re-presented to the hospital 4 weeks later with recurrent gastrointestinal bleeding. Repeat upper gastrointestinal endoscopy revealed the presence of two large gastric AVMs with active bleeding (●" Fig.2), which were treated with APC. Of note, these two AVMs had not been present on the previous endoscopy 4 weeks earlier. Despite treatment, the patient continued to experience gastrointestinal bleeding. 99mTechnetium-labeled red blood cell scintigraphy was performed, which revealed active bleeding in the jejunum consistent with the previously visualized AVM (●" Fig.3). Given the recurrent and diffuse nature of his AVMs across the entire gastrointestinal tract, he was treated with systemic pharmacologic therapy using octreotide. His gastrointestinal bleeding finally ceased after this octreotide therapy. He remains free of recurrent hemorrhage several months later.

Right heart dysfunction after left ventricular assist device implantation: a comparison of the pulsatile HeartMate I and axial-flow HeartMate II devices.
BACKGROUND Right heart dysfunction confers significant morbidity and mortality after left ventricular assist device implantation and historically occurs in as many as a third of patients. It is unknown whether newer axial flow pumps have a different impact on postimplant right heart dysfunction. We compared the incidence of right heart dysfunction after implantation of the pulsatile HeartMate I...

The HeartMate II continuous flow left ventricular assist device (LVAD) (Thoratec Inc) provides circulatory support for patients with medically refractory systolic heart failure as both a bridge to transplantation and destination therapy. Although left ventricular apical pseudoaneurysm has rarely been reported with first-generation pulsatile flow LVADs, this has not been previously reported with...
متن کاملA modified suturing technique for the implantation of the apical cannula of the HeartMate II left ventricular assist device.
The secure and haemostatic implantation of the left ventricular assist device (LVAD) apex inflow cannula can sometimes be challenging, especially in the setting of an acute myocardial infarction or significant ventricular wall thinning, when the myocardial tissue may be friable and tissue strength may be compromised. In these cases, surgical complications from this site may present a challenge....
متن کاملSurvival after left ventricular assist device with and without temporary right ventricular support.
BACKGROUND Right ventricular (RV) failure after the insertion of a left ventricular assist device (LVAD) historically results in poor outcomes. Patients requiring temporary RV support after LVAD insertion are a heterogeneous group of patients consisting of those in cardiogenic shock after myocardial infarction, to those with chronic decompensated heart failure. For patients requiring biventricu...

BACKGROUND Continuous-flow left ventricular assist devices (CF-LVADs) have become the standard of care for patients with end-stage heart failure (HF). While these devices have improved durability compared to earlier generation left ventricular assist devices (LVADs), increased frequency in some complications has been seen, including gastrointestinal bleeding (GIB), pump thrombosis and hemolysis...
